Independent Subcontractor Application

This application is for consideration of independent subcontractor status with the Company.

The Subcontractor expressly represents and warrants to the Contractor that:

He/she is not, and shall not be construed to be, an employee of the Contractor. The Subcontractor’s status shall be that of an independent subcontractor, solely responsible for his/her own actions and inactions.

The Subcontractor shall act solely as an independent subcontractor, and not as an employee, partner, or agent of the Contractor.

The Subcontractor is not authorized to enter into contracts or agreements on behalf of the Contractor, nor to create any obligations of the Contractor to third parties.

All subcontractors are required to complete this application. The information provided will be kept confidential and used solely to determine qualifications for subcontractor status.
